If you are a NEW PATIENT or its has been 2 years+ since your last visit, please call our office to book your consultation. If it has been 1-2 years since your last visit or if this is a NEW COMPLAINT since your last visit, please book a 20 minute appointment (“Re-Evaluation”) to give the doctor sufficient time to assess & diagnose. If it has been >1 year since your last visit but you still book a “Subsequent visit”, you will be charge accordingly. Thank you for your cooperation.
